Name Change FAQ for Uttar Pradesh

A correction means the record was always meant to say your name and somebody typed it wrongly, which usually needs only a marksheet and an affidavit. A change means you are now called something different, which needs publication. The difference is roughly Rs 1,500 and several weeks, so settle it first on our correction versus change page.

Covering letter, notarised affidavit, Public Notice in two typed copies with two witnesses on each, a CD in MS-Word format with its certificate, both complete newspaper pages, identity and address proof, photographs and the BharatKosh receipt.

Because that is what the department requires, and an unsigned second copy is one of the commonest reasons a file comes back.

Family members are perfectly acceptable. Each gives signature, full name and address, on both copies.

It states on oath that two names belong to one person, quoting a document somebody else can check. The wording is your responsibility, not the notary's.

No. They attest that you swore it. A badly worded affidavit gets stamped just as readily as a good one.

No law requires it, and keeping your maiden name causes fewer problems rather than more.
