- Both newspaper pages, complete and original, one Hindi and one English daily, masthead and date visible. Never clippings. The wording and paper choices are on the newspaper advertisement page.
- The typed proforma (deed change form), filled with your details and signed by you and two adult witnesses, everyone signing in the old name, with the witnesses' names and addresses.
- A signed declaration that the contents of the file are true, with your old and new names and your father's or husband's name and address.

The Minor Checklist: What Changes
For a child under 18, the file is the same shape with four differences:
- A parent or legal guardian executes everything: the affidavit, the proforma and the declaration are in the guardian's voice, declaring the child's name change.
- The child's birth certificate joins the file as a self-attested copy.
- The guardian's photo ID is enclosed alongside.
- The government fee is Rs 1,700 on BharatKosh instead of Rs 1,100.
No court order is needed for an ordinary minor name change, and the witnesses requirement stays the same.
Case Add-Ons: Marriage, Divorce, NRI, Government Employees
- After marriage: add the marriage certificate copy. If the marriage is not yet registered, that registration (igrsup.gov.in in UP) usually comes first.
- After divorce: add the divorce decree copy for reverting to the maiden name.
- Religious conversion: add the conversion certificate where one exists.
- NRIs from UP: the same file with the deed usually attested at the Indian embassy or consulate, and the fee at Rs 3,500.
- Central government employees: a different file entirely. Under MHA O.M. No. 190016/1/87-Estt. dated 12 March 1987, the affidavit is replaced by a deed attested by a Gazetted Officer, and the service book entry closes the loop.
The Three Rules That Reject Files
- The one-year rule: the affidavit and the newspaper ads must be under one year old on the date the file reaches Delhi. A stale paper means a fresh affidavit, fresh ads, and a fresh start.
- The never-returned rule: submitted documents, including your original newspaper pages, are not sent back. Photocopy or photograph every page before posting; those copies are your only record.
- The old-name rule: every signature in the file, yours and both witnesses', goes in the old name, because the new name has no legal existence until publication.
Documents You Do NOT Need
Half the panic in this process comes from lists padded with papers nobody asks for. For an ordinary adult name change through the Central Gazette, you do not need:
- A birth certificate (adults prove identity with any one photo ID; the birth certificate joins only a minor's file)
- School or board certificates (they get updated after the gazette, they do not feed it)
- A police verification or character certificate
- A court order (ordinary name changes never see a courtroom)
- A CD or soft copy disc (older guideline copies still float around the internet mentioning CDs and demand drafts; in current filing practice, payment is online on BharatKosh and the paper file is what travels)
- Any purchased application form (the proforma is a free format to type, not a form anyone sells)
The Keep-Copies Habit
Before the envelope closes: photocopy or photograph every page of the file, including both full newspaper sheets, the notarised affidavit and the signed proforma. Nothing comes back, and months later, when an office wants to see "what you submitted", these copies are the only answer you will have. Keep them with the courier receipt and the BharatKosh receipt in one folder until every document update is done.
What Other Websites Get Wrong About the Documents
- Wrong: attach newspaper clippings. Right: the complete original pages, masthead and date visible. Clippings are a standard reason files come back.
- Wrong: include a CD with your application in MS Word. Right: that requirement belongs to older guideline copies still circulating online. In current practice the fee is online on BharatKosh and the paper file is what counts.
- Wrong: you can go personally and submit at the gazette office. Right: the route is postal, in your own name, and agents are barred. There is no counter waiting for walk-ins.
