If your name is correct everywhere except one document, you likely need a correction at the office that issued that document, not this 7-step process. A birth certificate spelling error goes to the local Registrar under Section 15 of the RBD Act. A wrong letter in one UP Board marksheet goes to the board. Step 1: Prepare the Notarised Affidavit
The affidavit is the legal seed of the whole process. The gazette is supporting proof, not a correction. It rewrites no register on its own, so after publication you apply to each office, in this order: Aadhaar first (the base document, and remember it allows only two name changes in a lifetime), then PAN (a correction; the number never changes), then passport (always a reissue with a new number; visas do not transfer), then bank (a branch visit is unavoidable), then everything else.
