So the first question is never "which Noida office". It is: WHICH DISTRICT ISSUED THIS PAPER? Give both addresses. In a migrant city the present address and the permanent one are rarely the same, and naming the board that issued your marksheet does most of the work of explaining which district holds your older records.

Each witness gives signature, full name and address, and both sign both copies. Colleagues and neighbours in Noida are entirely acceptable; witnesses do not need to be family.
