The Process, Portal to Dispatch

1. Sequence the proofs first

   Gazette published where needed, then Aadhaar, then PAN, so the passport application lands with matching IDs. The sequencing logic lives on our process guide.

2. Apply for reissue on Passport Seva

   Register on the Passport Seva portal, choose "Apply for Fresh Passport / Reissue of Passport", and select "Change in Existing Personal Particulars" with the name field.

3. Enter the new name exactly

   Letter for letter as your gazette or certificate shows it, with the surname and given-name fields filled the way you want them printed for life.

4. Pay and book the PSK slot

   Pay the reissue fee online and book the nearest UP PSK or POPSK appointment.

5. Attend with originals

   Old passport, reason documents (certificate or Annexure J, or Annexure E with ads and gazette), two IDs in the new name, originals plus self-attested copies.

6. Police verification, then dispatch

   Verification runs at your current UP address, and the new booklet posts to you. Keep the old cancelled passport forever; visas and history live in it.

When the Gazette Comes First

For a full name change, the passport office is the strictest audience in India, and the gazette notification is the document it trusts: publish first, update Aadhaar and PAN with it, then file the reissue with the whole set matching. 1. Execute the affidavit precisely

   Stamp paper, notary, old and new names, address, reason. The PSK will compare your passport application against this wording, so a stray initial here becomes an objection there.

2. Advertise in two newspapers and keep both issues

   Hindi daily plus English paper, names and address in each, full original pages preserved. The passport file later wants these very clippings, which is a second reason never to lose them.

3. Finish the proforma, witnesses and CD

   Computer-typed “I hitherto known as”, two witnesses signing with details, CD in the prescribed format with its signed printout. The Department rejects more files on this step than any other.

4. Settle Rs 1,100 through BharatKosh

   Adult fee, receipt saved; a minor’s application through a guardian runs Rs 1,700. These figures are public, which makes inflated “government fee” quotes easy to catch.

5. Courier the bundle to the Department of Publication

   Civil Lines, New Delhi, with ID, address proof and photographs riding along. Applicants from Lucknow to Ballia file identically; the counter is a post office, not Delhi.

Gazette Fees, Time and the File

The gazette file itself needs: the notarised affidavit, both complete original newspapers, the typed proforma signed by the applicant and two witnesses, the CD with its signed printout, photo ID, address proof, recent photographs and the BharatKosh receipt.

What Other Websites Get Wrong About Passport Name Change

• Wrong: the passport office updates your name on the existing passport. Right: it is a reissue with a new booklet and a new number, which is why timing around visas matters.
• Wrong: a gazette notification is needed for the surname after marriage. Right: the marriage certificate or Annexure J carries that case. The gazette belongs to full name changes.
