Documents Required for Name Change After Marriage in Uttar Pradesh

You want to know what to carry, and a flat list is exactly why brides reach a counter and find something missing. The truth is that your document set changes twice: once depending on whether you take the marriage certificate route or the gazette route, and again depending on which desk you are standing at. This page gives the core set everyone needs, the extras each route adds, and a counter-by-counter table of what to carry to Aadhaar, PAN, the bank, the passport office and the rest.

Quick answer: Everyone needs the same core set: the marriage certificate (get at least three certified copies), your Aadhaar and PAN in the maiden name, your husband's identity document, address proof, passport photographs and a notarised affidavit. After that the list splits. The certificate route, which most married women in Uttar Pradesh use, adds nothing further. The gazette route adds two newspaper advertisements, a typed proforma with two witnesses, a CD and the BharatKosh receipt. Then each counter wants its own combination, which is what the table on this page is for.

The reason document lists fail is that they are written as one list. In practice your set changes twice. First by route: if your marriage certificate carries the update, your file is small, and if a passport office or a strict employer wants a gazette, it grows. Second by counter: the Aadhaar Seva Kendra, your bank branch and the Passport Seva Kendra each want a different combination, and one of them keeps your document rather than returning it. This page separates all three layers, and it starts with the document everything else depends on.

The Marriage Certificate, and Why Three Copies

This is the single most useful practical tip on the page and it costs almost nothing to follow. Apply for at least three certified copies of your marriage certificate at the time you collect it. The reasons are cumulative: several counters retain the copy you hand over, some institutions will not accept a plain photocopy or a scan where a certified copy is specified, and returning to the registering authority months later for another copy is a wasted day. Where the marriage has not yet been registered in Uttar Pradesh, that is the first thing to fix, because the certificate is what makes every later step short. Registration runs through the state's registration system, and the certificate issued there is the document Aadhaar, banks and the passport office will each want to see. Two further notes. Check the spelling of both names on the certificate itself before you leave the counter, because a certificate carrying an error is worse than no certificate at all. And keep one certified copy permanently unused, filed with your other originals, so you always have a clean one in reserve.

What Each Route Adds

RouteAdditional documentsWho takes this route
Certificate routeNothing beyond the core setMost married women in UP: Aadhaar, PAN, bank, employer and insurance all update on the certificate and affidavit
Gazette routeTwo original full newspaper pages, typed proforma in two copies with two witnesses, CD with signed declaration, BharatKosh receipt for Rs 1,100Anyone facing a passport office, a central recruitment, a university outside UP or a strict employer
Court routeNot applicable to an ordinary marriage name changeOnly where a dispute exists, which is rare in this context

The honest position, and the one that saves money: start on the certificate route unless something specific has told you otherwise. If a passport application is in your near future, take the gazette route from the beginning rather than doing both, and read our Central versus State comparison before choosing where to publish. The full gazette envelope, item by item, sits on our gazette documents guide.

Counter by Counter: What to Carry

Where you are goingCarryWhat they keep
Aadhaar Seva KendraMarriage certificate, existing Aadhaar, husband's Aadhaar, address proofCopies; originals are returned after verification
PAN correctionUpdated Aadhaar, marriage certificate, existing PAN detailsHandled online, so uploads rather than counter copies
Bank branchCertified marriage certificate copy, affidavit, updated Aadhaar and PAN, request letterOne certificate copy for the KYC file
Passport Seva KendraJoint affidavit, marriage certificate, spouse's passport where applicable, existing passport, and both newspaper pages if a gazette was usedCopies, plus the gazette printout
Employer, PF and payrollAffidavit, certificate copy, updated AadhaarOne set for the service record
Insurance and investmentsCertificate copy, updated PAN, the policy or folio numberOne set per policy or folio
Tehsil certificatesUsually nothing extra once Aadhaar carries the new nameFresh certificates are issued rather than edited

Two habits make this round painless. Carry a self-attested photocopy set of everything in one folder rather than assembling it each morning, and update in the order that avoids repeat visits: Aadhaar first, then PAN, then the rest, as our marriage journey pillar sets out.

Originals, Photocopies and Self-Attestation

If the Marriage Was Never Registered

This is common in Uttar Pradesh, where a marriage solemnised by rites years ago may never have been entered into any register, and the discovery usually happens at a bank counter. Two things are true at once. Your name change is still possible: the affidavit route works, using the version that states no certificate has been obtained and adds that the maiden and present names belong to one and the same person, as set out on our affidavit guide. And registering the marriage is still worth doing, because the certificate is what makes the passport leg, and several institutional processes, straightforward rather than argumentative. If a passport application is anywhere in your plans, treat registration as the first task rather than the last.

Two Claims About the Gazette That Are Simply Wrong

Several pages ranking for this topic repeat two statements that will cost you time if you believe them.

Both errors point the same way: they make the gazette sound like a letter you request. It is a publication you apply for, and the difference is a full envelope of documents. Our download guide explains what actually arrives, and what does not.

Central Gazette Name Change: Step by Step

Our core service: where your case needs the gazette rather than the certificate route, this is the filing we do every week, and the document set above becomes the envelope below.

  1. Swear the affidavit

The standard name change affidavit on stamp paper carrying the maiden name, the married name, your address and the reason, notarised in your own district.

  2. Publish two newspaper advertisements

One Hindi daily of your district (Dainik Jagran, Amar Ujala or Hindustan) and one English paper, with complete original pages kept flat, because the passport office wants them again later.

  3. Type the proforma and prepare the CD

Two typed copies with two witnesses giving their signature, full name and address, and the CD carrying the same declaration in Word without witness details.

  4. Pay Rs 1,100 on BharatKosh

Before dispatch, since the printed receipt is an enclosure inside the envelope.

  5. Courier the file to Civil Lines

Tracked dispatch to the Department of Publication, Delhi 110054, with your phone number and email on the covering letter.

  6. Download from Part IV and update

Publication in 25 to 45 days, free download, three prints, then Aadhaar, PAN, bank, passport and the certificate family in that order.

Gazette Fees, Time and the File

Marriage certificate copiesNominal per certified copy; take three
AffidavitRs 100 to 500 including stamp paper and notary
Adult gazette feeRs 1,100 on BharatKosh, only on the gazette route
Newspaper advertisementsA few hundred rupees per paper, edition-dependent
Certificate route totalUsually just the certificate copies and the affidavit
Publication window25 to 45 days after the Department receives the file
Our serviceBasic from Rs 999, Complete from Rs 1,999, quoted separately

The Department's checklist in one line: covering letter with phone and email, affidavit (notarised), newspapers (both, original, whole pages), proforma (typed, two copies, applicant plus two witnesses), CD with signed declaration, photo ID, address proof, photographs, BharatKosh receipt.

Does the Marriage Certificate Go Into the Gazette Envelope?

Short answer: not as a required item, but include it anyway. The Department's checklist asks for the affidavit, both newspapers, the proforma, the CD, identity and address proof, photographs and the fee receipt. A marriage certificate is not on that list, because the gazette does not care why you are changing your name, only that the declaration is properly made. In practice we enclose a copy regardless, for one reason: it makes the file internally coherent. A reader who sees a maiden name becoming a married surname, with a certificate showing exactly that, has no question to ask.

ItemMarriage case
AffidavitThe gazette version, sworn by you alone, not the joint marriage affidavit
Newspaper advertisementsBoth papers, wording matching the affidavit; the marriage is the reason, stated once
ProformaTyped, two copies, with the relationship line reading wife of, not daughter of
Address proofUse your marital address if that is where you now live and can document it
PhotographsYour own passport-size photographs, not the joint ones from the marriage affidavit
Marriage certificateNot required, but worth enclosing as a copy for coherence

The relationship line deserves the emphasis. Most married applicants have spent their whole life on forms as daughter of, and the proforma for a post-marriage change reads wife of, which people fill in on autopilot and then have to retype. The field-by-field specimen is on our proforma guide.

After the Gazette: What Updates Next

Aadhaar at the Seva Kendra first, then PAN online, then your bank branch, then the passport with both newspaper pages if a gazette was involved. After that the Tehsil certificate family, education records, driving licence, voter ID, and the two records people forget for years: the employer service record with PF nomination, and any insurance policy or mutual fund folio still in your maiden name.

Marriage Name Change Documents: FAQs

What documents do I need to change my name after marriage?

The core set is the marriage certificate, your Aadhaar and PAN in the maiden name, your husband's identity document, address proof, photographs and a notarised affidavit. The gazette route adds two original newspaper pages, a typed proforma with two witnesses, a CD and the BharatKosh receipt.

How many copies of the marriage certificate should I get?

At least three certified copies. Several counters keep the copy you hand over, some institutions refuse plain photocopies where a certified copy is specified, and returning to the registering authority later is a wasted day.

Do I need my husband's documents?

Yes, a self-attested copy of his Aadhaar or passport is part of the core set, because the relationship is what the name change rests on. The passport leg may also want his passport where applicable.

Is the gazette compulsory after marriage?

No. Most married women in Uttar Pradesh complete every update on the marriage certificate and an affidavit. The gazette becomes necessary where a passport office, a central recruitment, a university outside UP or a strict employer insists.

Will the gazette notification be posted to my address?

No. Printed sale of gazette copies was discontinued in favour of electronic publishing, so the notification appears in Part IV online and you download it free. Pages claiming registered-post delivery in 15 to 20 days are simply wrong.

Can I start the gazette by writing a letter with my certificate?

No. A gazette application is a full envelope: notarised affidavit, both original newspaper pages, typed proforma in two copies with two witnesses, CD with a signed declaration, photographs, identity and address proof and the fee receipt.

What if our marriage was never registered?

The affidavit route still works, using the version that states no certificate has been obtained and adds that the maiden and present names are one and the same person. Registering the marriage remains worthwhile, especially if a passport application is coming.

Do photocopies need to be self-attested?

Yes, sign across every photocopy before handing it over. An unsigned photocopy is a small gap that sends people back to a photocopy shop mid-visit.

Which counter should I visit first?

The Aadhaar Seva Kendra. PAN, banks, the passport office and the Tehsil certificate family all read Aadhaar, so updating it first prevents repeat visits everywhere else.

What does Aadhaar accept as proof after marriage?

The marriage certificate issued by the registering authority is the recognised document. Carry your existing Aadhaar, your husband's Aadhaar and the address proof you are using consistently.

My name is spelled wrong on the marriage certificate. What now?

Fix that first, at the authority that issued it. A certificate carrying an error will propagate into every record you update from it, which is far more expensive to unwind than correcting it now.

What does the whole document set cost?

Certified certificate copies at nominal rates, the affidavit at Rs 100 to 500, and nothing else on the certificate route. The gazette route adds Rs 1,100 to BharatKosh plus a few hundred rupees per newspaper insertion.

Facts on this page are checked against these sources and our own filing experience. Counter practice varies at the margins; the core set, the route split and the certified-copies rule are the stable core.

The Short Version

Core set for everyone: marriage certificate with three certified copies, your Aadhaar and PAN in the maiden name, your husband's identity document, address proof, photographs and a notarised affidavit. The certificate route adds nothing further and covers most married women in Uttar Pradesh. The gazette route adds two original newspaper pages, a typed proforma with two witnesses, a CD and the Rs 1,100 receipt. Each counter then wants its own combination, and several keep a copy rather than returning it, which is why three certified copies matter. If your marriage was never registered, the affidavit still works with the one-and-the-same-person clause. And no, nothing arrives by registered post: the gazette publishes online in 25 to 45 days.
