Quick answer: Everyone brings the same core: the old passport in original, proof of your current address, and self-attested copies of everything. On top of that sits one document that explains your change, and which one depends on your reason: a marriage certificate or Annexure J, a divorce decree, or a gazette notification. Where a gazette applies, the passport office also wants both complete newspaper pages. A downloaded gazette PDF is fine and needs no seal, because that is how gazettes are now issued.

The Digital Gazette Question

The worry we hear most often on this subject, and it is worth answering plainly.

Concern "My gazette is just a PDF I downloaded. There is no stamp, no seal, no signature. Will the passport office accept it?" Answer Yes. That is how the Gazette of India is issued now. There is no sealed paper version to get, no attestation to obtain, and no office that stamps it. The download IS the document.

What the counter looks at is whether the notification is genuine, which it verifies from the issue details rather than from any seal. So print it cleanly, keep the issue number and date noted separately, and do not pay anybody to have it attested, because there is nothing to attest and no authority that does it.

Print about five copies while you are at it. Several offices keep one, and the download is free and permanent, so the only thing you cannot do is print more while standing in a queue. Our download guide covers finding your entry in Part IV.

Why This Counter Wants Your Newspapers

Every other office accepts the gazette alone: Aadhaar Kendra gazette Bank gazette Board gazette Tehsil gazette The PASSPORT office asks for: the gazette notification AND both complete newspaper pages Complete pages. Masthead and date visible. Not the clipping.

This catches more people than anything else in the whole passport process, because by the time you apply the advertisements are six weeks old and newsagents do not stock back issues. Some newspaper offices will supply an archived copy, which is slow and occasionally impossible for a small district edition.

The fix costs ten rupees and happens on the day the advertisement runs: buy two copies of each edition. Our newspaper guide makes the same point, and it is the single most repeated piece of advice we give.

Self-Attestation, Done Properly

People sometimes pay a notary to attest passport copies out of caution. It is not required for a reissue and it is not harmful, but the money is unnecessary, and knowing that saves a small amount on a day when other things cost more.

Stage 1: The affidavit

Keep this affidavit after publication rather than discarding it. Some counters ask to see it alongside the gazette, and it costs nothing to have in the folder.

Stage 2: The Public Notice, in two copies

PUBLIC NOTICE (TWO identical typed copies, each signed and witnessed) It is for general information that I, ____________________ S/O ____________________ , R/o ______________________________ ____________________________________ , declare that I have changed my name from ____________________ to ____________________ and I shall hereafter be known as ____________________ for all purposes. Each witness gives signature, full name and address, and both sign both copies at one sitting.

Stage 3: The CD and CD-Certificate

CD-CERTIFICATE I, ____________________ S/O ____________________ , R/o ____________________________________________________ , do hereby declare:- 1- That the matter in CD and hard copy is the same and I am responsible for any mismatch. 2- That I am responsible for the quality of CD. 3- That the CD is in MS-Word Format. 4- That the above statement is true and correct. Open it on another computer and check the text can be selected.

Stage 4: Newspapers, fee, covering letter and dispatch

  1. Publish in two newspapers and buy four copies

    One Hindi daily of your district and one English paper. Two copies of each edition: one pair for the Department, one pair kept for the passport office later.

  2. Pay Rs 1,100 on BharatKosh

    Before the envelope is sealed, with the function head, PAO and DDO checked on the confirmation screen and two receipts printed.

  3. Write the covering letter

    To The Controller, Publication Department, Civil Line, Delhi 110054, saying the formalities are complete and the documents are enclosed, with your mobile number and email.

  4. Assemble, check and photocopy

    Affidavit, Public Notice in two witnessed copies, CD with CD-Certificate, both newspaper pages, identity and address proof, photographs and the receipt, with a full copy set at home.

  5. Courier it, tracked

    One to three days from most UP districts, with no counter to visit.

  6. Download, print five, then book the Kendra

    Publication in 25 to 45 days. Download the whole issue, note the issue number and date, print about five copies, and only then take an appointment.
