What matters is your service book, the departmental record that follows you from joining to pension, because it does not update itself when a notification is published. Quick answer: Do the gazette exactly as anyone else would, then give your office a written intimation with a copy of the notification and ask for it to be recorded in your service book. That entry is the one that matters, because your service book governs promotion, transfers and above all your pension papers, and it does not update itself. Ask for written confirmation that the entry has been made, and keep it. Then update your salary account and other records to match.

Government service adds one step to this process and it is the step nobody outside government thinks about. A private employee who changes their name updates HR and the matter ends. In service, there is a bound record of your entire career that has to be amended by your own establishment section, and the consequence of it not happening does not appear for twenty years.

What a Service Book Is

Your service book is the departmental record of your whole career: appointment and confirmation pay fixation and increments promotions and transfers leave account and your NAME Everything the department later needs to know about you is drawn from it, including your pension papers. It is not connected to the gazette. It is not connected to Aadhaar. Somebody in your office writes in it.

That last line is the whole point of this page. The Gazette of India publishing your notification changes nothing inside your establishment section's cupboard. A clerk has to make an entry, and clerks make entries when they receive a written request with a reference, not when something happens in Delhi.

Writing the Intimation

  1. Address it through proper channel

    To your Head of Office or Head of Department, in the form your department normally uses.

  2. State both names and the gazette details

    Old name, new name, the gazette issue number and date, so nobody has to search for anything.

  3. Give your employee and service book number

    Which is what lets the establishment section find your record on the first attempt.

  4. Request the entry explicitly

    Ask in words for the change to be recorded in your service book. A covering note that only encloses the gazette often gets filed rather than actioned.

  5. Attach the gazette and the newspapers

    Printed copies. Some departments want the newspaper pages too, which is another reason to have bought spares.

  6. Ask for written acknowledgment of the entry

    Not that the letter was received, but that the entry has been made. These are different things and only the second protects you.

The shape that works: To: The Head of Office / Department (through proper channel) Subject: Change of name, service book entry I, ____________ , [designation], employee number ____________ , have changed my name from ____________ to ____________ , published in the Gazette of India, Part IV, issue ____________ dated __-__-____ . I request that the change be recorded in my service book and in departmental records, and that written confirmation be issued. Enclosures: gazette copy, newspaper pages, affidavit copy.

Why Pension Depends on It

None of this is dramatic and all of it is avoidable with one letter and one acknowledgment. Put the acknowledgment somewhere you will still have it in thirty years, along with the gazette itself.

The Order of Work

1. GAZETTE affidavit, newspapers, publication 25 to 45 days 2. INTIMATION to your office written, with the gazette attached ask for the SERVICE BOOK entry 3. Written acknowledgment keep this permanently 4. Salary account so credits match the service book 5. Aadhaar, PAN, passport, the rest in the usual order Steps 2 and 3 are the ones only government employees have. They are also the ones people skip.

Your identity documents can run alongside from step four onwards, since they do not depend on the departmental entry. What should not happen is finishing every personal document and never sending the letter, which is exactly the pattern that produces a pension problem later.

If the Change Follows a Marriage

If It Was Never Recorded

  1. Ask to see or confirm your service book entry

    Employees are entitled to know what their record says, and asking early is far easier than asking late.

  2. Send a fresh written request

    With the gazette copy again, even if you sent it years ago, and a reference to the earlier letter if you kept one.

  3. Explain the gap plainly

    That the change was published on a date and appears not to have been recorded, without accusing anybody of anything.

  4. Escalate through proper channel if needed

    In writing, to the next level above your establishment section, as our problems page sets out.

  5. Get the acknowledgment this time

    Confirming the entry, not the receipt of your letter.

  6. Keep everything together

    Gazette, both letters, and the acknowledgment, in one place, permanently.

This is much easier to fix at year five than at year thirty, and easier at year thirty than during pension processing. If you are reading this and are not sure whether your own entry was ever made, that is worth one letter this week.

Central Gazette: the Complete Process and File Format

Stage 1: The affidavit

AFFIDAVIT (on non-judicial stamp paper, notarised) I, ____________________ S/O ____________________ , R/o ____________________________________________________ , do hereby solemnly affirm and declare as under:- 1- That my name has been recorded as ____________________ in my ____________________ (service records and documents). 2- That I have changed my name from ____________________ to ____________________ and I shall hereafter be known as ____________________ for all purposes. 3- That both the said names are the name of one and the same person, that is, myself. 4- That it is my true and correct statement. DEPONENT VERIFICATION:- Verified at ____________ on this __-__-20__ that the contents of the above Affidavit are true and correct to the best of my knowledge and belief. DEPONENT

Some departments keep a copy of the affidavit alongside the gazette, so make one extra when you have it notarised rather than hunting for the notary again later.

Stage 2: The Public Notice, in two copies

PUBLIC NOTICE (TWO identical typed copies, each signed and witnessed) It is for general information that I, ____________________ S/O ____________________ , R/o ______________________________ ____________________________________ , declare that I have changed my name from ____________________ to ____________________ and I shall hereafter be known as ____________________ for all purposes. Each witness gives signature, full name and address, and both sign both copies at one sitting.

Stage 3: The CD and CD-Certificate

CD-CERTIFICATE I, ____________________ S/O ____________________ , R/o ____________________________________________________ , do hereby declare:- 1- That the matter in CD and hard copy is the same and I am responsible for any mismatch. 2- That I am responsible for the quality of CD. 3- That the CD is in MS-Word Format. 4- That the above statement is true and correct. Stage 4: Newspapers, fee, covering letter and dispatch

  1. Publish in two newspapers and buy four copies

    One Hindi daily of your district and one English paper. Two copies of each edition, because your department may want a set and so may the passport office.

  2. Pay Rs 1,100 on BharatKosh

    Before the envelope is sealed, with the function head, PAO and DDO checked on the confirmation screen and two receipts printed.

  3. Write the covering letter

    To The Controller, Publication Department, Civil Line, Delhi 110054, with your mobile number and email.

  4. Assemble, check and photocopy

    A full copy set at home, plus one extra affidavit copy for your department.

  5. Courier it, tracked

    One to three days from most UP districts, with no counter to visit.

  6. Publish, download, then send the intimation

    Twenty-five to forty-five days, then the download, then your written request for the service book entry.

Confirmation of the Entry, Not the Letter

Two different acknowledgments: "Your letter dated __-__ has been received." -> proves you wrote -> proves nothing about the record "The change of name has been recorded in the service book of the applicant." -> proves the entry exists -> this is the one worth having Ask for the second. In writing. And keep it with the gazette for the rest of your career. Thirty years from now this piece of paper will be worth more than anything else in the file.

Establishment sections are usually happy to issue it; the reason it is often missing is simply that nobody asked. Ask politely, once, in the original letter, and again if the reply only confirms receipt.
