Quick answer: Start with Aadhaar in her own name, because everything else follows from it. If she has no document of her own, there is a documented route: enrolment through the head of the family, using a proof of relationship such as a ration card or marriage certificate that names them both, with the head of family authenticating. The care of line is optional, so it need not be carried. Then a bank account in her own name, and outwards from there.

This is not a small group of people. In most villages here it describes a majority of women above a certain age.
The Care of Line Is Optional
- UIDAI has clarified that the care of field on Aadhaar is optional, not mandatory, so a father s or husband s name need not be displayed.
- Cards remain valid either way, whether that line is present or blank.
- So it can be left out at enrolment, or removed later, and nobody at a counter is entitled to insist otherwise.
- A viral claim that the field had been removed altogether was false, and the accurate position is simply that it was never compulsory.
- Check the current requirement at the centre before you go, since Aadhaar procedures are revised from time to time.

If She Holds Nothing of Her Own
Find a document that names her and a relative together
A ration card, a marriage certificate, a marksheet or a pension record. It does not have to be in her name alone; it has to show the relationship.
Identify the head of the family for this purpose
Somebody already enrolled who holds their own identity and address documents.
Take both of them to the enrolment centre together
The head of family authenticates and confirms her identity and address.
Give her name exactly as she wants it recorded
On a written slip, in capitals, and ask for it to be read back before anything is submitted.
Leave the care of line blank if she prefers
It is optional, and this is the one moment when choosing is easy.
Keep the acknowledgment and check the card when it arrives

If She Wants Her Maiden Name Back
- Nothing obliges a woman to carry a married surname, and nothing obliges her to keep one either.
- Where the change is genuine, the ordinary route applies: affidavit, two newspaper notices and the gazette.
- Where she simply never changed it, no gazette is needed at all, and what is required is that the documents stop disagreeing.
- After a divorce the decree itself is often accepted as the reason, and the route is set out separately.
- Decide the full name once and apply it everywhere, rather than changing one document and leaving the rest.

What Is Local in Mainpuri

Stage 1: The affidavit
Point one is unusual and it is the whole purpose: it states her own name first, before any relationship. Point two then lists where she currently appears, and point three deals honestly with the common situation of no registered birth. Read together, it establishes a person rather than describing a household.
Stage 2: The Public Notice, in two copies
A thumb impression is as good as a signature here, and the form allows for it. Each witness gives signature, full name and address, and both sign both copies. Family members are acceptable witnesses.

When a family brings us this kind of case, the papers usually arrive with a son or a nephew, and the conversation happens entirely between us and him. We stop and ask a simple question: what does she want her name written as.
