Form 8 for Voter ID Name Correction in Uttar Pradesh

Your name is spelled wrongly on the voter list and you want it corrected through Form 8. Three things set this apart from everything else in your file. The electoral roll is published for anybody to read. You may only be on it once, and a second entry somewhere else is not a nuisance but an offence. And unlike every other correction on this subject, it costs nothing.

Quick answer: Before filing anything, search the electoral roll for your own name in both places you have lived. If you appear twice, sort that out first, because a duplicate entry is a more serious matter than a spelling. If you appear once, file Form 8 through the voter services portal or the Voter Helpline app, attach a document showing the correct spelling, and expect the BLO to verify locally. It costs nothing.

Most pages here start with the process. This one starts with a check, because the commonest problem people bring to a voter list correction is not the one they came about.

Search Yourself First

Five minutes, before any form: search the roll for your name in the district you grew up in search again in the district you live in now search under BOTH spellings and search under your maiden name if you married What you are looking for is not the spelling. You are looking for how many YOUs there are. one entry -> file Form 8, straightforward two entries -> stop, and deal with that first Most people never think to look, because it does not occur to anybody that they might be on a list twice.

This happens most often after marriage, when a woman is enrolled at her husband's address by a family member filling in a fresh application, while the entry at her parents' address quietly stays where it is.

Why Two Entries Matter

Nothing on this site treats a document error as more than a nuisance. This is the exception, and it is worth saying plainly rather than burying in a FAQ.

A Record Anyone Can Read

DocumentWho can see it
AadhaarYou and offices you show it to
PANYou and the tax department
MarksheetYou and whoever you give it to
Bank recordYou and the bank
Electoral rollAnybody at all
The roll is published. That is the point of it: an election needs a list people can check. Which means a wrong spelling here is not sitting quietly in a file. It is on a document that neighbours, employers and anybody curious can look at, alongside your father's or husband's name and your address. That is not a reason to be alarmed. It is a reason to fix it, since this is one of the few places where an error is genuinely visible to people who are not offices.

Which Form Does What

  1. Form 6 enrols a new voter

For somebody not on any roll, which is not you if you already hold a card.

  2. Form 7 deletes an entry or objects to one

This is the form for removing a duplicate at an old address.

  3. Form 8 corrects entries and records a shift

Spelling, relative's name, address change and replacement of the card all sit here.

  4. So a correction is Form 8

Straightforwardly, when you appear on the roll once.

  5. And a move is also Form 8

Rather than a fresh Form 6, which is precisely how duplicates get created.

  6. Using Form 6 to move is the mistake

It enrols you afresh without removing anything, leaving two entries behind.

Say that last point again, because it causes most of the trouble on this page: MOVING HOUSE -> Form 8, shifting NOT -> Form 6, fresh enrolment Form 6 is for somebody who has never been on a roll. Using it to "register at the new address" is what leaves the old entry alive.

Filing Form 8

Deciding which spelling to submit is worth a moment, and our spelling correction page covers settling on one version rather than fixing documents one at a time forever.

What the BLO Does

Your application does not go to a distant office. It goes to a Booth Level Officer who covers your polling area. They may: visit the address ask a neighbour ask you to come to a nearby point with the original documents This is local work by design, because the roll is about who lives WHERE, and only somebody local can confirm that. So keep your phone reachable and the originals to hand for a few weeks after filing. A missed call is the commonest reason one of these stalls.

The offline route through the BLO directly, for people who would rather not use the portal, is a normal alternative rather than a fallback.

Central Gazette: the Complete Process and File Format

Our core service: a voter list correction needs no publication. Where a name is genuinely changing, this is the route from Lucknow.

Stage 1: The affidavit

AFFIDAVIT (on non-judicial stamp paper, notarised) I, ____________________ S/O ____________________ , R/o ____________________________________________________ , do hereby solemnly affirm and declare as under:- 1- That my name is recorded as ____________________ in my High School marksheet, Roll No. ____________________ . 2- That my name has been wrongly recorded as ____________________ in the electoral roll of ____________________ Assembly Constituency, EPIC No. ____________________ . 3- That ____________________ and ____________________ are the name of one and the same person, that is, myself. 4- That I am not enrolled as a voter at any other place. 5- That it is my true and correct statement. DEPONENT VERIFICATION:- Verified at ____________ on this __-__-20__ that the contents of the above Affidavit are true and correct to the best of my knowledge and belief. DEPONENT

Point four appears on no template we have seen and belongs here. Swearing that you are enrolled in one place only is exactly the assurance this particular record is concerned with, so check it is true before you sign it.

Stage 2: The Public Notice, in two copies

PUBLIC NOTICE (TWO identical typed copies, each signed and witnessed) It is for general information that I, ____________________ S/O ____________________ , R/o ______________________________ ____________________________________ , declare that I have changed my name from ____________________ to ____________________ and I shall hereafter be known as ____________________ for all purposes. It is certified that I have complied with other legal requirements in this connection. ____________________ Signature of Applicant WITNESS NO. 1 Signature :- ____________________ Full Name :- ____________________ Address :- ____________________ WITNESS NO. 2 Signature :- ____________________ Full Name :- ____________________ Address :- ____________________

Each witness gives signature, full name and address, and both sign both copies at one sitting.

Stage 3: The CD and CD-Certificate

CD-CERTIFICATE I, ____________________ S/O ____________________ , R/o ____________________________________________________ , do hereby declare:- 1- That the matter in CD and hard copy is the same and I am responsible for any mismatch. 2- That I am responsible for the quality of CD. 3- That the CD is in MS-Word Format. 4- That the above statement is true and correct. Signature of Applicant ____________________ R/o ____________________

Stage 4: Newspapers, fee, covering letter and dispatch

  1. Publish in two newspapers and buy four copies

One Hindi daily of your district and one English paper, complete original pages.

  2. Pay Rs 1,100 on BharatKosh

With the function head, PAO and DDO checked and two receipts printed.

  3. Write the covering letter

To The Controller, Publication Department, Civil Line, Delhi 110054, with your mobile number and email.

  4. Assemble, check and photocopy

A full copy set kept at home.

  5. Courier it, tracked

One to three days from most UP districts.

  6. Then Aadhaar, then the voter list

Which is free, so it can be done at any point after.

The One That Costs Nothing

Worth noticing, because it is unusual here. gazette Rs 1,100 newspapers a few hundred each affidavit Rs 100 to 500 RTO fee modest Tehsil applications small passport reissue significant VOTER LIST nothing at all No fee, no stamp paper, no counter payment. An application from a phone and a visit from a BLO. So of everything on a post-marriage or post-gazette list, this is the item with no financial reason to delay it. The only cost is remembering.

Which makes it a good one to do first among the free-standing tasks, rather than last as usually happens.

What Other Websites Get Wrong

Areas We Serve in Uttar Pradesh

Your entry belongs to an assembly constituency and a polling booth, so this is local work everywhere. Across all 75 districts, most often these:

Booth Level Officer
The local election official who handles voter roll corrections offline.
Form 8
The election form used to correct details on a voter record.
Name correction
Fixing a name that was written wrongly by somebody else. Lighter than a change, because an older document already shows the right version.
Department of Publication
The office at Civil Line, Delhi that receives name change files and publishes the notification.
Name change
Adopting a different name from the one your documents correctly record. This is the case that usually needs publication.

Whichever counter you reach next, there is a guide for it here:

Why We Search Before We File

Somebody asks for help with a spelling on a voter card and we spend the first five minutes searching the roll in two districts instead. About a third of the time we find a second entry nobody knew about, usually created years earlier by a relative meaning well. Correcting the spelling without dealing with that would leave the real problem in place and neatly spelled. Vipin Chauhan, B.Tech LL.B, looks before filing.

Form 8 Name Correction: FAQs

What should I do before filing?

Search the electoral roll for yourself in every district you have lived in as an adult, under both spellings and under a maiden name if you married.

Why does that matter?

Because being enrolled in more than one constituency is an offence, and a correction filed on top of a duplicate leaves two entries with one correctly spelled.

How do duplicates happen?

Usually by accident, when somebody enrols afresh at a new address using Form 6 instead of recording a shift on Form 8.

Which form corrects a spelling?

Form 8, which covers correction of entries and shifting of residence. Form 6 enrols a new voter and Form 7 deletes an entry.

What does it cost?

Nothing. This is the only correction on this subject with no fee at all.

Is the electoral roll really public?

Yes. It is published so that people can check it, which means a wrong spelling here is visible in a way that a bank record is not.

What spelling should I submit?

The one on your Class 10 marksheet, and upload the marksheet as the supporting document.

Who verifies my application?

A Booth Level Officer covering your polling area, who may visit the address or ask you to come to a nearby point with originals.

Why did my application stall?

Most often a missed call. Keep the phone number on the form reachable for a few weeks after filing.

How long does it take?

Weeks, driven by the BLO visit and the revision cycle the application is processed against.

Do I need a gazette?

No. A voter list correction involves no publication.

Can I do it offline instead?

Yes, through the BLO directly. That is a normal route rather than a fallback.

Facts here are checked against these sources and our own casework across the state. Forms and portal design are revised from time to time, so follow the current instructions on the official portal over any description here.

About the founder: Founded by Vipin Chauhan, B.Tech LL.B, with 5+ years of direct filing experience with the Department of Publication. Every process detail here comes from files we have actually prepared and followed through to publication, not from copied web content.

The Short Version

Three things make the voter list unlike anything else in your file. It is published, so a wrong spelling is not sitting quietly in an office but on a document anybody can read alongside your address and your father's or husband's name. You may only be on it once, and a second entry in another constituency is an offence rather than the kind of untidiness a spelling on a bank record is. And it is free to correct, the only item on a post-marriage or post-gazette list with no financial reason to delay. That combination changes where to start. Not with the form, but with a search: look for yourself in every district you have lived in as an adult, under both spellings and under a maiden name. Duplicates are common, they are almost always accidental, and they come from using Form 6 to register at a new address when Form 8 records a shift.

Moved After Marriage?

Then check both constituencies before filing anything. Two entries is the problem worth catching first.
