Search Yourself First
This happens most often after marriage, when a woman is enrolled at her husband's address by a family member filling in a fresh application, while the entry at her parents' address quietly stays where it is.
Why Two Entries Matter
- Being enrolled in more than one constituency is an offence, not a paperwork untidiness like a spelling on a bank record.
- It usually happens by accident, through a fresh enrolment rather than a shifting application, and nobody intended anything.
- The fix is a deletion of the old entry, which is a different form from the one you came here about.
- Doing it yourself, promptly, is much better than having it found later by somebody else.
- And a correction filed on top of a duplicate just leaves you with two entries, one of them now correctly spelled.
Nothing on this site treats a document error as more than a nuisance. This is the exception, and it is worth saying plainly rather than burying in a FAQ.

Which Form Does What
Form 6 enrols a new voter
For somebody not on any roll, which is not you if you already hold a card.
Form 7 deletes an entry or objects to one
This is the form for removing a duplicate at an old address.
Form 8 corrects entries and records a shift
Spelling, relative's name, address change and replacement of the card all sit here.
So a correction is Form 8
Straightforwardly, when you appear on the roll once.
And a move is also Form 8
Rather than a fresh Form 6, which is precisely how duplicates get created.

It enrols you afresh without removing anything, leaving two entries behind.
