Cleared, Then Stopped.

Name Change in Azamgarh

This district sends enormous numbers of young people into competitive examinations, and there is one thing about names that almost nobody is told in time. A mismatch between your form and your marksheet does not stop you sitting the exam. You will sit it, and you may clear it. It stops you later, at document verification, which is the cruellest possible moment to find out. And where a commission uses a one time registration, the name you enter once carries forward to every future application.

Quick answer: On any examination form, enter your name exactly as it appears on your Class 10 marksheet, not as it appears on Aadhaar and not as your family says it. Where a commission uses a one time registration, that entry becomes the name on every future application, so it is worth ten careful minutes. If a mismatch already exists, fix it between cycles, while nothing is pending, rather than after a result.

Nothing in this is difficult. It is simply a thing people find out at the worst possible time.

The Exam Lets You Through

Follow what actually happens. YOU FILL THE FORM the name is whatever you typed THE ADMIT CARD ARRIVES it carries that same name YOU SIT THE EXAM the invigilator matches your face to the photograph, not your spelling to a marksheet THE RESULT COMES you have cleared DOCUMENT VERIFICATION and here, for the first time, somebody places your form beside your marksheet Every stage before that last one is silent about the name. So a person can prepare for three years, clear a written examination, tell their family, and then be stopped at a table by a difference of one letter. That is why this is worth reading ahead of a result rather than after one.

Preparing for that table is a two minute job at the form stage and a months long job afterwards, which is the entire argument.

Match the Marksheet, Not Aadhaar

What people copy fromWhat verification compares against
Aadhaar, because it is in handThe Class 10 marksheet
An older applicationThe Class 10 marksheet
What the family calls themThe Class 10 marksheet
Memory, on the last dayThe Class 10 marksheet
This is the mistake that costs most, and it comes from a reasonable instinct. Aadhaar is the document everybody has in their pocket. It is the one used at a bank, a SIM counter, a hostel. So it feels like the identity document. For recruitment it is not the senior one. The Class 10 marksheet is what establishes your name and date of birth for these purposes, and it is what a verification table opens first. So when you fill a form: put the marksheet on the table type from it, letter by letter then check Aadhaar against what you typed That order matters. Most people do it the other way round, and that is precisely how the difference gets in.

Where Aadhaar itself is the odd one out, the update route is on our Aadhaar page, and the limits there are worth reading before you attempt it.

One Registration, Used For Years

Think about what this actually means. An eighteen year old registers once, in a hurry, typing from memory. At twenty three, having cleared something, he reaches a verification table and discovers that every application he has ever made carries the same small difference. Not one form. All of them. That is not a disaster and it can be dealt with. But it is a great deal of trouble created by ten unhurried minutes not taken. If you have not registered yet, this paragraph is the most useful thing here. If you already have, open the account today and read what it says, while nothing is pending.

If a Mismatch Already Exists

  1. Work out which document is actually wrong

The marksheet, or everything else. That decides whether this is a board correction or an ordinary update.

  2. If the marksheet is right, correct the rest to match it

Aadhaar first, then whatever else. This is the easier direction by a long way.

  3. If the marksheet itself is wrong, that is the slow route

It goes through your own school to the board, so start it early and do not wait for a vacancy.

  4. Do it between cycles, with nothing pending

When no result is awaited and no verification is scheduled.

  5. Keep an affidavit linking the two versions

Cheap, quick, and it answers the question at a table if one is ever asked.

  6. Then update the registration account itself

Following whatever the commission s own procedure is, and keeping the confirmation.

The board correction route in full is on our UP Board page, and what to do if a school will not forward it on our school refuses page.

Never Change It Mid Process

One warning, and it matters more than anything else on this page. If a recruitment is already running, do not change your name in the middle of it. you applied as one name the admit card says that name the result will say that name the verification will expect that name Introducing a third version halfway through turns a simple check into a question about your identity, at exactly the moment you can least afford one. Finish the process. Then change the name. The only exception is where the commission itself asks you to correct something, in which case follow their instruction and keep it in writing. This is the same rule that applies to a passport during an employment abroad: correct timing beats correct paperwork.

The passport version of that rule is on our Sultanpur page.

Central Gazette: the Complete Process and File Format

When is a gazette needed here? Where the name genuinely changed. Where a form simply differs from a marksheet, an affidavit linking the two is usually what a verification table wants.

Stage 1: The affidavit

AFFIDAVIT (on non-judicial stamp paper, notarised) I, ____________________ S/O ____________________ , R/o Village ____________________ , Tehsil ____________________ , District Azamgarh, Uttar Pradesh, do hereby solemnly affirm and declare as under:- 1- That my name is recorded as ____________________ in my High School marksheet, Roll No. ____________________ , issued by ____________________ Board in the year ______ . 2- That my name is recorded as ____________________ in my application / registration bearing No. ____________________ for ____________________ . 3- That both the said names are the name of one and the same person, that is, myself, and that my correct name as per my High School marksheet is ____________________ . 4- That it is my true and correct statement. DEPONENT VERIFICATION:- Verified at ____________ on this __-__-20__ that the contents of the above Affidavit are true and correct to the best of my knowledge and belief. DEPONENT

Point two names the application or registration number and the recruitment, and point three ends by naming the marksheet as the authority. Written this way, the affidavit answers the exact question a verification table asks, rather than making a general statement about your name.

Stage 2: The Public Notice, in two copies

PUBLIC NOTICE (TWO identical typed copies, each signed and witnessed) It is for general information that I, ____________________ S/O ____________________ , R/o ______________________________ ____________________________________ , declare that I have changed my name from ____________________ to ____________________ and I shall hereafter be known as ____________________ for all purposes. It is certified that I have complied with other legal requirements in this connection. ____________________ Signature of Applicant WITNESS NO. 1 Signature :- ____________________ Full Name :- ____________________ Address :- ____________________ WITNESS NO. 2 Signature :- ____________________ Full Name :- ____________________ Address :- ____________________

Only where the name is genuinely changing, and only between cycles. Each witness gives signature, full name and address, and both sign both copies.

Stage 3: The CD and CD-Certificate

CD-CERTIFICATE I, ____________________ S/O ____________________ , R/o ____________________________________________________ , do hereby declare:- 1- That the matter in CD and hard copy is the same and I am responsible for any mismatch. 2- That I am responsible for the quality of CD. 3- That the CD is in MS-Word Format. 4- That the above statement is true and correct. Signature of Applicant ____________________ R/o ____________________

Stage 4: Newspapers, fee, covering letter and dispatch

  1. Publish in two newspapers and buy six copies

A Hindi daily and an English paper covering this district, complete original pages.

  2. Pay Rs 1,100 on BharatKosh

    Online, with two receipts printed.

  3. Write the covering letter

To The Controller, Publication Department, Civil Line, Delhi 110054, with your mobile number and email.

  4. Assemble, check and photocopy

A full copy set kept at home, because the file sent is never returned.

  5. Courier it, tracked

A day or two from here to Delhi.

  6. Then Aadhaar, then the registration account

In that order, and keep every confirmation.

Ten Minutes, Not the Last Day

Before you submit any examination form, do this once. [ ] marksheet open on the table [ ] type the name from it, not from memory [ ] type the father's and mother's names from it too [ ] type the date of birth from it [ ] read the whole form back once [ ] screenshot it before final submission Then, only then, compare Aadhaar against what you typed. If they differ, you have found something valuable on a day when it costs nothing. Do this on an ordinary evening, well before the closing date. Forms filled on the last night, from memory, on a phone, at a cyber cafe with a queue behind you, are where almost every one of these problems begins. Ten minutes. Once. Years of consequence.

Key Terms on This Page

Plain definitions for the words that come up at the counter:

Affidavit
A statement you swear is true, signed before a notary on stamp paper. The notary confirms you signed it, not that the contents are true.
Department of Publication
The office at Civil Line, Delhi that receives name change files and publishes the notification.
Name correction
Fixing a name that was written wrongly by somebody else. Lighter than a change, because an older document already shows the right version.
Name change
Adopting a different name from the one your documents correctly record. This is the case that usually needs publication.

Whichever counter you reach next, there is a guide for it here:

Why We Ask Whether Anything Is Pending

When a young person from this district contacts us about a name, our first question is whether any recruitment is currently running for them. If one is, our advice is usually to wait, finish it, and change the name afterwards, because introducing a third version of a name halfway through a selection process creates a question about identity at the worst possible moment. If nothing is pending, this is the right week and we say so. We also ask what their registration account says, because that is the entry that quietly repeats itself across years of applications and almost nobody checks it. Telling somebody to wait is not what they expect from a service, but it is the right answer more often than not. Vipin Chauhan, B.Tech LL.B, asks what is pending first.

Name Change in Azamgarh: FAQs

Will a name mismatch stop me sitting the exam?

Usually not. You will sit it and may clear it. It stops you at document verification, which is why it is dangerous rather than obvious.

Which document should my form match?

The Class 10 marksheet. That is what a verification table opens first, not Aadhaar.

But Aadhaar is my main identity document.

For a bank or a SIM, yes. For recruitment the marksheet is the senior one for name and date of birth.

What is a one time registration?

An account created once, from which every later application draws. The name you enter there follows you for years.

I registered years ago in a hurry.

Open the account today and read what it says, while nothing is pending. That is the cheapest moment to find a difference.

A recruitment is running right now. Should I fix it?

No. Finish the process first. A third version of your name halfway through creates a question about identity at the worst moment.

What if the commission itself asks me to correct something?

Then follow their instruction and keep it in writing. That is the exception.

My marksheet is the one that is wrong.

That is the slower route, through your own school to the board. Start it early rather than waiting for a vacancy.

Do I need a gazette?

Only if the name genuinely changed. For a form that differs from a marksheet, an affidavit linking the two is usually enough.

What should the affidavit say?

Both versions, the application or registration number, and that the marksheet name is the correct one.

When should I fill a form?

On a calm evening well before the closing date, with the marksheet open, not at midnight from memory.

What should I keep with my marksheet?

The affidavit, any gazette and the newspaper pages, in one folder. That is the folder you will carry to a verification table.

Official Sources

Facts here are checked against these sources and our own casework in Purvanchal. Requirements differ between commissions and change between cycles, so read the notification for the recruitment you are actually applying to.

About the founder: Founded by Vipin Chauhan, B.Tech LL.B, with 5+ years of direct filing experience with the Department of Publication. Every process detail here comes from files we have actually prepared and followed through to publication, not from copied web content.

The Short Version

Follow what actually happens when a name on a form differs from the name on a marksheet. You fill the form and the name is whatever you typed. The admit card arrives carrying that same name. You sit the exam, where an invigilator matches your face to a photograph rather than your spelling to a certificate. The result comes and you have cleared. Then, at document verification, somebody places your form beside your marksheet for the first time. Every stage before that last one is silent about the name, which is exactly why this catches people who prepared for three years. So type the name from the marksheet rather than from Aadhaar, do it on a calm evening rather than the last night of the window, and treat a one time registration as the important form, because the name entered there follows you across years of applications. And if a recruitment is already running, finish it before changing anything.

Filling a Form This Cycle?

Send the name on your marksheet and the name you were about to type. Two minutes now, before the window closes.
