Deciding it once, in a single sentence, at the kitchen, is the whole of the prevention.
The First Form Decides It
- Whatever is entered on the first official form becomes the reference, because everything afterwards is checked against it.
- That form is usually an Aadhaar enrolment or a school admission, and often the child is too small to say anything about it.
- The person filling it writes what they hear or what they know, which may be the called name, a short form, or a description.
- Nobody flags it at the time, because at that moment nothing depends on it.
- Years later everything depends on it, and by then two or three documents have copied each other.

Full name, spelling, and whether a surname is used. Write it on a slip in capitals.
List every document the child already has
Birth certificate, Aadhaar, school record, and anything else, with what each one says.
Decide which document will be the reference
Normally the birth certificate if there is one, otherwise the school admission record.

Not spread over two years, because each office compares against the others and half-done is worse than not started.
Do the same for every child in the house
The same person enrolled them, so the same problem usually repeats across siblings.

For a child below Class 10 this is usually unnecessary. Correct the school record instead. The gazette matters where a name is genuinely being changed, and for a minor the fee is higher.
Stage 1: The affidavit, sworn by the guardian
Point three is the one that does the work, and it appears in this form for a reason. It says plainly that one name is the household name and the other is the formal one, rather than pretending an error occurred.
