Nothing about that is dramatic. It is simply how the document works, and it explains why an insurer looks harder at a name on a claim form than a bank looks at one on a passbook.

  4. Take the original policy document

  5. Take identity proof and the correct spelling

  6. Write a short request letter

    Naming the policy number, both spellings and what you are asking for.

People with several policies bought at different times often find they have two or three servicing branches, which is worth mapping once rather than discovering one at a time.

Naming the servicing branch as well as the policy number is worth doing here, because this affidavit may be read years later by somebody who has never seen the file and needs to know where it belongs.

Stage 2: The Public Notice, in two copies

Each witness gives signature, full name and address, and both sign both copies at one sitting.

Stage 3: The CD and CD-Certificate

Stage 4: Newspapers, fee, covering letter and dispatch

  1. Publish in two newspapers and buy four copies

    One Hindi daily of your district and one English paper, complete original pages.

  2. Pay Rs 1,100 on BharatKosh

    With the function head, PAO and DDO checked and two receipts printed.

  3. Write the covering letter

  4. Assemble, check and photocopy

    A full copy set kept at home.

  5. Courier it, tracked

    One to three days from most UP districts.

  6. Then Aadhaar, then the bank, then the policies

Make a List While You Are At It

One sheet of paper, kept with the policies: INSURER POLICY NO. Then tell one family member the sheet exists and where it is. That sheet does more good than any single correction on this page, and it takes about fifteen minutes.
