Quick answer: Write the affidavit as a supporting document, not the proof. It should state the recorded date, the correct date, and above all name the record you are relying on, since that record is what actually carries the file. Never write that you declare your date of birth to be something, because declaring does not make a birthday. Four formats follow, for a registrar's office, a Board application, an Aadhaar counter, and the case where two of your own documents show different dates.
Every other affidavit on this site does real work. A name affidavit can settle a question on its own, because a name is something you use and answer to and you are a competent witness to it. A date is different. You were present at your birth but you were not observing, and nobody expects you to have been. So an office reading your affidavit is not looking for your assertion; it is looking for what you point at. Write it that way and it helps. Write it as a declaration and it quietly weakens the file.

Key Takeaways
- An affidavit supports a date file; the early record is what proves it.
- The most important clause is the one naming that record, with its number and issuer.
- Declaring a date rather than reporting one reads as an assertion and weakens the application.
- A Board affidavit needs your examination, year and roll number, or the office cannot locate you.
- Two documents showing different dates need a one-and-the-same clause, the same as two names would.

Why It Cannot Prove a Date
This is why applicants sometimes feel their perfectly drafted affidavit was ignored. It was not ignored; it was weighed and found to contain no evidence. The registrar or the Board was waiting for the sentence that points at a hospital slip or an admission form, and that sentence was not there.
What It Can Actually Do
- Identify the wrong entry precisely, by record number and issuing office, so nobody has to search for you.
- Name the record you rely on, which is the clause that turns a claim into a comparison.
- State both dates exactly, as printed, removing any ambiguity about what is being asked.
- Confirm one-and-the-same person where two documents carry different dates for you.
- Carry the request, saying plainly which field you want corrected and why.
- Establish standing, where a parent or guardian is swearing it for a minor.
Format One: For the Registrar
Where you are asking the birth registrar to correct the register entry itself.
Clause two is doing all the work, so fill it properly: name the document type, the issuing office and its number. "As per my other records" is worthless here. The wider approach is on our birth certificate date page, including which early records carry weight.
Format Two: For a Board Application
Same substance, plus the identifiers an examination office needs before it can find you in a register.
Clause one is why this format exists. A Board does not begin by weighing your claim; it begins by locating your entry, and without the examination, year, roll number and registration number that is slow work. Note also that the Board accepts date corrections only within a limited window from passing, which our Board date page sets out.
Format Three: For an Aadhaar Counter
Shorter, because the Kendra is looking mainly at your birth certificate and the affidavit is context rather than argument.
Remember that a date update spends your only lifetime allowance on Aadhaar, so the affidavit is the least important thing you carry that day. What matters is that the birth certificate is already correct and that you read the operator's screen before biometrics, as our Aadhaar date page explains.
Format Four: Two Documents, Two Dates
For the situation where you are not correcting anything yet, but need to satisfy a counter that two records showing different dates belong to one person.
This is the date equivalent of the one-and-the-same-person affidavit used for names, and it serves the same purpose: getting you past a desk today while the underlying correction is arranged. It is a bridge rather than a solution, and the sooner the actual record is fixed the better.
The Fatal Wording, and Three Lesser Ones
| Mistake | Why it matters |
|---|---|
| "I declare my date of birth is __-__-____" | Presents hearsay as a declaration. Report the date and name the record instead |
| "As per my other documents" | Names nothing, so the reader has no record to check. Give the type, issuer and number |
| Explaining how the error occurred | Nobody is deciding that, and speculation about a 1994 form invites questions |
| Bundling a name change into the same affidavit | Different routes, different offices. A mixed affidavit gets rejected whole |
That last one deserves emphasis because it happens often. A name and a date frequently go wrong together, and it is natural to want one document covering both. Keep them separate: the name half may need a gazette and the date half never can, so a single affidavit ends up asking two offices for incompatible things.
Central Gazette: the Complete Process and File Format
Our core service: a date never goes through publication. This is the file for the name half, which so often travels with a date problem, drafted so it can be sworn at the same notary sitting.
Stage 1: The gazette affidavit
Notice how different this is from the four formats above. Here you genuinely are declaring something, and the words "shall hereafter be known" are doing exactly what they say. That verb has no equivalent for a date, which is the whole reason the two problems travel on separate tracks.
Stage 2: The Public Notice, in two copies
Each witness gives signature, full name and address, and both sign both copies at one sitting. This is also the one document in the whole set that needs witnesses at all.
Stage 3: The CD and CD-Certificate
Stage 4: Newspapers, fee, covering letter and dispatch
Publish in two newspapers
One Hindi daily of your district and one English paper, booked the day the affidavit is sworn, keeping complete original pages with masthead and date, and buying two copies of each.
Pay Rs 1,100 on BharatKosh
Before the envelope is sealed, with the function head, PAO and DDO checked on the confirmation screen and two receipts printed.
Write the covering letter
To The Controller, Publication Department, Civil Line, Delhi 110054, saying the formalities are complete and the documents are enclosed, with your mobile number and email.
Assemble, check and photocopy
Affidavit, Public Notice in two witnessed copies, CD with CD-Certificate, both newspaper pages, identity and address proof, photographs and the receipt, with a full copy set at home.
Courier it, tracked
One to three days from most UP districts, with no counter to visit.
Search Part IV after 25 to 45 days
Weekly Gazette, Part IV, a Saturday date range covering your window, then download the whole issue.
Draft Both, Swear Both, One Sitting
Tell the notary you need two documents rather than one long one. It costs a little more in stamp paper and saves a great deal in rejected applications, and it means the date file can move while the gazette is still in its publication window.
Gazette Fees, Time and the File
| Affidavit | Rs 100 to 500 including stamp paper and notary |
| Two affidavits, one sitting | Roughly double the stamp paper, one trip |
| Registrar correction | The registering office's application fee |
| Board application | Around Rs 1,000, inside the Board's window |
| Aadhaar date update | A nominal Kendra fee, one lifetime allowance |
| Adult gazette fee | Rs 1,100, for a name only, never a date |
| Publication window | 25 to 45 days after the Department receives the file |
The envelope in one line, for the name half: covering letter with your phone and email, affidavit (notarised), Public Notice typed in two copies with two witnesses on each, CD in MS-Word format with the signed CD-Certificate, both complete newspaper pages, photo identity proof, address proof, photographs and the BharatKosh receipt.
Before You Apply: Check These
- Make sure the affidavit names your early record by type, issuer and number.
- Never write that you declare your date of birth; report it and point at the proof.
- Add the examination, year and roll number for any Board application.
- Keep the name affidavit and the date affidavit as separate documents.
After the Gazette: What Updates Next
An affidavit is never the end of a date case, only the paper that accompanies one. Once the underlying record is corrected, take the fresh document to whichever office was asking, and keep the affidavit filed alongside the early record you relied on. Both come back into use at verifications years later, because a changed date is questioned long after a changed name has stopped attracting attention.
What Other Websites Get Wrong
- Wrong: an affidavit can establish your date of birth. Right: it supports a file; the early record proves the date.
- Wrong: "I declare my date of birth is..." is standard wording. Right: it presents hearsay as a declaration and weakens the file.
- Wrong: one affidavit can cover a name and a date. Right: they go to different offices and a mixed document gets rejected whole.
- Wrong: a longer affidavit is stronger. Right: the clause naming your record is the document; the rest is housekeeping.
- Wrong: affidavits need witnesses. Right: only the gazette Public Notice does.

Why We Ask Which Record You Hold
On a date affidavit our question is never what you want it to say. It is which early document you have, because that determines the only clause that matters. If the answer is none, the affidavit is not the problem and we say so rather than drafting something that will not work. Vipin Chauhan, B.Tech LL.B, writes these to point rather than to assert.
- We build the affidavit around your record: naming its type, issuer and number.
- We keep declaration wording out: because it reads as hearsay and weakens the file.
- We split name and date into two documents: sworn at one sitting so they stay consistent.
- We tell you when no affidavit will help: which is when there is nothing to point at.

Date of Birth Affidavit: FAQs
Can an affidavit prove my date of birth?
No. You were present at your birth but not observing it, so what you write is something you were told. An affidavit supports the file; the early record proves the date.
What is the most important clause?
The one naming the record you rely on, with its type, issuing office and number. Everything else in the document is housekeeping.
What wording should I avoid?
"I declare my date of birth is..." Report the date instead and point at the record: "my correct date of birth is __-__-____, as recorded in..."
Is "as per my other documents" acceptable?
No. It names nothing, so the reader has no record to check. Give the document type, the issuer and the number.
What extra does a Board affidavit need?
Your examination, year, roll number and registration number, because the office locates your entry in a register before it assesses anything.
Can one affidavit cover my name and my date?
No. They go to different offices and say different things, and a mixed affidavit tends to be rejected as a whole. Draft two and swear them at the same notary sitting.
Do I need witnesses?
No. An affidavit is signed by the deponent before a notary. Only the gazette Public Notice needs two witnesses giving signature, full name and address.
Can I sign it before visiting the notary?
No. The signature must be made in the notary's physical presence, and an affidavit signed beforehand is not properly executed.
What if I have no early record at all?
Then the affidavit is not what is missing. Search the family papers for an immunisation card or hospital slip first, because without something to point at no wording will carry the file.
Who swears it for a child?
The parent or guardian, with a clause establishing the relationship, since a minor cannot swear their own affidavit.
What does it cost?
Rs 100 to 500 including stamp paper and notary, varying by district. Two affidavits at one sitting cost roughly double the stamp paper and one trip.
Does an affidavit help at an Aadhaar counter?
Only as context. The Kendra is looking at your birth certificate, and the update spends your one lifetime date allowance, so the certificate matters far more than the wording.
Official Sources
- Affidavit formats published by Indian Missions and professional bodies, which use the same reporting structure rather than a bare declaration.
- The Registration of Births and Deaths framework, under which a register entry is amended by the registrar on documentary evidence.
- UPMSP correction practice, in which an affidavit accompanies rather than replaces a documented application.
- UIDAI's update framework, under which a date of birth may be updated once in a lifetime against acceptable proof.
Facts here are checked against these sources and our own drafting experience. Receiving authorities occasionally prescribe their own wording, which always takes precedence; the fact that an affidavit supports rather than proves a date is the stable part.
The Short Version
An affidavit about a date cannot do what an affidavit about a name does. You are a competent witness to your own name because you use it and answer to it, but you were not observing your own birth, so a sworn statement about the date is something you were told rather than something you know. Offices read it that way whether or not they say so. What the document can do is identify the wrong entry precisely, state both dates exactly as printed, and above all name the early record you are relying on, with its type, issuing office and number. That single clause is the affidavit; everything else is housekeeping. Never write that you declare your date of birth to be something, and never bundle a name change into the same document, because the name half may need a gazette and the date half never can.
