WRONG "I have changed my surname from VERMA to SINGH." A counter reads this and asks: whose? There are thousands of Vermas becoming Singhs. Nothing here is you. RIGHT "I have changed my name from PRIYA VERMA to PRIYA SINGH and I shall hereafter be known as PRIYA SINGH." Now the notification names a person, and that person is matchable to your Aadhaar, your marksheet and your passport application.

The instinct behind the short version is reasonable: only one part changed, so why write the rest. But a gazette entry is not a form with fields, it is a public statement about a named individual, and a statement that does not name the individual cannot do its job.

Use the same discipline in the affidavit and in the newspaper advertisements. All three should carry the identical full name on both sides, because a counter that finds three slightly different versions asks questions that a consistent set never provokes.
